Valid Passport

The same passport used to complete the C5 Form. Must be valid for the full duration of your intended stay in Jamaica.

C5 Form QR Code

Shown on your phone or printed on paper. Sent to your email immediately after form submission. Save it before you travel.

Return or Onward Ticket

Proof of Accommodation

Hotel booking confirmation or host's address. This should match what you entered on your C5 Form.

Proof of Sufficient Funds

Do Children Need Their Own C5 Form?

Yes - every traveler needs their own individual Jamaica C5 Form. This includes infants and young children. There is no age exemption.

Parents or legal guardians are responsible for completing the form on behalf of minors. When filling in the form, simply enter the child's personal details exactly as they appear on their passport.

One form per family is not accepted. Each traveler - including newborns - must have a separate, completed form.
