The Jamaica C5 Form is a mandatory online entry form for all travelers arriving in Jamaica, also called the Electronic Passenger Declaration. It has been required since September 1, 2023, when Jamaica fully replaced its paper arrival card.
The C5 Form combines your immigration declaration and customs declaration into a single online form. It is completed at the official portal - enterjamaica.gov.jm - and is completely free of charge.
Most travelers complete the form in 5 to 10 minutes. You will receive a QR code by email immediately after submitting. You must present that QR code - along with your passport - to the immigration officer on arrival.
There is no exemption for any traveler entering Jamaica. Every person - regardless of nationality, age, or purpose of travel - must complete their own individual C5 Form before arrival.
This is unique compared to many other countries' entry forms. Even Jamaican citizens and permanent residents must submit the form each time they return.

Go to enterjamaica.gov.jm and create an account or continue as a guest. Enter your passport details, flight information, accommodation address, and customs declaration.
Your full name must match your passport exactly - including any middle names. Double-check your passport number character by character before submitting.
After you submit the form, a QR code is sent to your email immediately. No waiting period - there is no approval process.
Save this QR code on your phone or print it before you travel. Do not delete this email - you will need the QR code to enter Jamaica.
Show your QR code and passport to the immigration officer on arrival. This applies at all airports including Sangster International (MBJ) and Norman Manley (KIN).
Cruise passengers must also present their QR code when disembarking at any Jamaican seaport.
Complete your C5 Form before departure - free Wi-Fi kiosks are available at MBJ but relying on them is risky.
Jamaica offers stunning beaches, vibrant culture, world-famous music, and warm hospitality. Whether you're heading to Montego Bay, Negril, Kingston, or exploring the Blue Mountains, your trip begins with the C5 Form.
The C5 Form is mandatory for every international visitor. Complete it online before your flight - free, instant, and takes just 5-10 minutes. You'll receive your QR code by email immediately.

The form is managed jointly by PICA (Passport, Immigration and Citizenship Agency) and the Jamaica Customs Agency (JCA). It is completed at enterjamaica.gov.jm.
Every traveler entering Jamaica must complete the C5 Form - with no exceptions. This includes tourists, business travelers, returning residents, and Jamaican citizens. It also applies to cruise ship passengers, children, and infants.

Yes. The Jamaica C5 Form is completely free at the official government portal - enterjamaica.gov.jm. There is no cost. The Jamaica Customs Agency and PICA do not charge any fee for this form.
Many unofficial third-party websites charge between $50 and $100 to complete this form on your behalf - these sites are not official and you should never pay to complete this form.
You can submit the form up to 30 days before your arrival date. We recommend completing it 24 to 48 hours before departure, once your flight details are confirmed.
Submitting with an incorrect or outdated flight number is one of the most common mistakes - always confirm your final flight before submitting.
No. The C5 Form is not a visa. It is a pre-arrival declaration required of all travelers - even those who do not need a visa to enter Jamaica.
If your nationality requires a Jamaican visa, you must obtain that separately through the Jamaican consulate or at pica.gov.jm. Then you must also complete the C5 Form separately.
Yes. All cruise ship passengers who disembark and clear immigration in Jamaica must complete the C5 Form. This applies at all major Jamaican seaports including Port of Montego Bay, Port of Kingston, Ocho Rios Cruise Terminal, Falmouth Cruise Terminal, and Port Antonio.
If you are a cruise passenger who stays on board and does not clear immigration, you do not need to complete the form.
